European Radiology:ER +乳腺癌缺氧和血管功能的PET/MRI评估

磁共振成像(MRI)和正电子发射断层成像(PET)已被用于探测体内乳腺癌微环境的病理生理学方面,包括灌注和缺氧。

乳腺癌中,微环境的病理生理学特征是不规则的血管网络导致灌注异常和缺氧。转录因子缺氧诱导因子1α(HIF-1α)是细胞适应缺氧的主要调节因子,支持血管生成和肿瘤的代谢重构,使其处于对氧气和营养物质依赖较少的状态。在各种癌症中,缺氧与肿瘤的侵袭性、抗治疗性和转移有关,并且是导致雌激素受体阳性(ER +)乳腺癌患者临床预后不佳的一个关键因素。以前的研究表明,在氧气水平降低的情况下,缺氧相关蛋白(包括HIF-1α及其下游靶点碳酸酐酶IX(CAIX))的过度表达与雌激素受体-α(ER-α)水平被抑制、去分化表型、对内分泌治疗的抵抗、乳腺癌复发和无病生存期缩短相关。

虽然一些专家已经讨论了HIF-1α和CAIX在ER+乳腺癌中的潜在临床价值,但对这种癌症类型的体内肿瘤病理生理学和低氧调节蛋白的表达之间的关系还没有充分探索。磁共振成像(MRI)和正电子发射断层成像(PET)已被用于探测体内乳腺癌微环境的病理生理学方面,包括灌注和缺氧。现阶段动态对比增强(DCE)MRI已被广泛用于临床研究以确定肿瘤血管功能的特征,而使用[18F]标记的硝基咪唑的PET,如[18F]-氟咪唑([18F]-FMISO)可以提供细胞内缺氧的模式评估。 [18F]-FMISO-PET已显示出对ER+肿瘤患者的预后潜力,以及预测对初级内分泌[20]和抗血管生成治疗的反应的效用。然而,尽管缺氧在ER+疾病中具有预后意义,但在ER+乳腺肿瘤中,将肿瘤微环境的功能参数与组织病理学证据或其他生物标志物相关的临床影像学研究非常有限


近日,发表在European Radiology杂志上的一篇研究将[18F]-FMISO-PET和DCE-MRI的体内成像参数与ER+乳腺癌的缺氧和血管的免疫组织化学标志物联系起来探讨了其在临床评估上的应用及价值。

本项研究对在本机构内年龄大于18岁、经活检确认、治疗无效的原发性ER+乳腺癌患者进行了手术前[18F]-FMISO-PET/MRI检查。血管功能的参数由DCE-MRI得出,使用扩展的Tofts模型,而缺氧则使用[18F]-FMISO流入率常数Ki进行评估。组织学肿瘤切片用CD31、缺氧诱导因子(HIF)-1α和碳酸酐酶IX(CAIX)进行了染色肿瘤微血管的数量、血管中位直径和微血管密度(MVD)由CD31免疫组化获得。HIF-1α和CAIX的表达使用组学分数来评估,组学分数由染色的阳性细胞百分比乘以染色强度取得。 

在检查的病变中,14/22(64%)为导管癌,2级或3级(19/22;86%),其中17/22(77%)HER2阴性。[18F]-FMISO Ki与血管直径(p = 0.03)、MVD(p = 0.02)和CAIX表达(p = 0.002)呈负相关,而DCE-MRI药代动力学参数和免疫组化变量之间没有发现明显关系。HIF-1α与任何PET/MR成像指标没有明显相关性。

本项研究表明,由[18F]-FMISO-PET测量的缺氧水平与ER+乳腺癌中CAIX表达增加、MVD低和血管直径较小具有相关性,进一步证实了ER+乳腺癌中血管不足和缺氧之间的相关性。
