微卫星不稳定性(MSI)是肿瘤学中一种新兴的可操作表型,能够提示肿瘤对免疫检查点通路免疫治疗的反应。。但是,目前仍缺乏低成本,高度准确和可跨癌症类型普遍化的MSI诊断。研究人员使用单分子分子倒位探针(smMIPs)开发了一种针对MSI的泛癌信息的多种微卫星基因座靶向高通量测序方法。 研究人员设计了一个跨肿瘤类型的针对可提供信息111个位点的smMIP。同时利用smMIP介导的纠错技术开发了一个分析框架,不需要输入匹配的正常材料可特异且灵敏地检测不稳定事件。 研究发现,使用合成的DNA混合物,smMIP对至少1%MSI阳性细胞敏感,并且在重复之间一致性很高。所鉴定的不稳定微卫星的部分可以以高准确度区分显示MSI的肿瘤与缺乏MSI的肿瘤,且敏感性和特异性分别为:直肠(100%诊断灵敏度和特异性),前列腺(100%诊断敏感度和特异性)和子宫内膜癌(95.8%诊断敏感性和100%特异性)。MSI-PCR是目前用于MSI的分子诊断标准,对结直肠肿瘤同样有效,但在前列腺(81.8%的诊断敏感性和100%的特异性)和子宫内膜(75.0%的诊断敏感性和100 %特异性)肿瘤中有一定的假阴性。 研究表明
