Front Endocrinol:中国学者研究---2型糖尿病患者振动感知阈值增高与肾功能下降的关系

较高的VPT与T2DM患者CKD的发病率呈正相关,特别是UACR升高。VPT可作为肾小球和肾小管损伤的标志。

背景:慢性肾脏疾病(CKD)是糖尿病患者死亡的主要原因,其特点是GFR的进行性下降、出现蛋白尿和不可逆的终末期肾脏疾病(ESRD)。然而,在临床实践中,由于缺乏对肾小球和肾小管对肾功能潜在影响的认识,可能会错过最佳的治疗时机。因此,在临床实践中,对糖尿病肾病进行系统的评估,不仅对发现早期疾病,而且对监测其向晚期进展速度至关重要。

振动感觉阈值(Vibratory sensory threshold, VPT)是定量感觉检查的重要指标,主要指示有髓鞘的Aa和Ab感觉神经纤维的病变。先前的研究表明,VPT在临床或初级保健中可以快速准确地识别糖尿病周围神经病变(DPN),反映大神经纤维的损伤。由于其无创性、易用性和患者依从性高,VPT检查越来越多地用于糖尿病并发症的临床研究。近年来的临床研究强调了VPT在DPN和糖尿病足溃疡诊断中的临床意义。此外,它还可以作为评估糖尿病患者非酒精性脂肪性肝病的心脏自主神经病变、冠状动脉疾病、脑血管疾病和肝纤维化的指标。这些发现表明VPT具有多方面的临床应用价值。此外,尿微量白蛋白(MA)和免疫球蛋白G (U-IgG)排泄率已被广泛认为是肾小球病变的标志物,而a1-微球蛋白(a1-MG)具有肾小球损伤标志物的特征。

目的:本研究通过对T2DM患者肾小球和肾小管的评估,探讨了VPT水平与肾功能障碍之间的关系。我们期望这项研究有助于在T2DM患者中发现新的和有前途的CKD标志物。

方法:选取2017年1月至2020年6月在福建医科大学第一附属医院内分泌科就诊的T2DM患者1274例。将患者按VPT水平进行分组,分为正常VPT组(VPT<15V)、轻度-中度VPT升高组(VPT15~25V)、重度VPT升高组(VPT≥25V) 3组,采用线性相关分析VPT与肾功能的相关性,包括肾小球标志物MA、U-IgG、a1-MG。CKD是根据肾脏疾病改善全球结局(KDIGO)标准定义的。对VPT与CKD、eGFR<60 ml/min、UACR >30 mg/g的关系进行二元logistic回归分析。

结果:轻、中度、重度VPT升高组与正常VPT组比较,肾小球损伤生物标志物(MA、U-IgG)、肾小管损伤生物标志物(a1 - MG)、CKD发生率、eGFR<60 ml/min、UACR > 30 mg/g逐渐升高。此外,与VPT正常的患者相比,糖尿病和严重升高的VPT患者的MA (b=197.54, p=0.042)和a1-MG (b=11.69, p=0.023)水平显著升高。此外,轻度至中度VPT升高患者的MA水平也显著升高(b=229.02, p=0.005)。轻中度VPT升高组患者(OR=1.463, 95% CI 1.005-2.127;OR=1.816, 95% CI 1.212-2.721)和VPT严重升高组(OR=1.704, 95% CI 1.113-2.611;OR=2.027, 95% CI 1.248-3.294)与VPT正常组相比,CKD发病率更高,UACR水平升高>30mg/g。此外,VPT重度升高组Upro阳性发生率显著高于对照组(OR=1.738, 95% CI 1.182 ~ 2.556)。然而,在eGFR <60 ml/min的发生率中没有观察到这种现象。
