由一名MRI医师和一组6人的口腔医师共同开发MRI检测裂纹/折裂存在与否的标准。29颗成人牙齿因临床诊断为牙根裂纹/折裂而拔除,并匹配29个对照。样本通过体内MRI协议及参照标准进行扫描(ie, 体外CBCT影像的限制性视野)。通过单盲的4人小组评估及比例随机重复来建立介入可靠性。计算每个显象模式的整体观察者一致性、敏感性和特异性。

结果显示,主观上讲,MRI能够增加裂纹/折裂的反差,相对于CBCT成像,不太容易产生来自放射性材料的伪影。MRI的介入可靠度从普通到极佳(κ = 0.38-1.00),而CBCT影像从中等到极佳(κ = 0.66-1.00)。MRI的特异性为0.83 (95% CI, 0.64-0.94; P < .01),而CBCT 影像为0.90 (95% CI, 0.73-0.98; P < .01)。

结论:尽管MRI增加了反差并且放射性材料不存在伪影,但是敏感性和特异性比较结果显示(to limited field of view CBCT imaging),MRI还需要改善其诊断准确性,特别是在图像采集和后处理参数上。基于技术研发的早期阶段,MRI还是可以被用来诊断牙齿的裂纹/折裂。
