In the initial stages of disease, levodopa is the most effective therapy for improving motor symptoms in Parkinson ’ s disease (PD). However, long - term treatment is accompanied by the development of fl uctuations in motor performance, dyskinesias, and neuropsychiatric complications. Furthermore, as PD progresses, patients develop features that do not respond well to levodopa therapy, such as freezing episodes, autonomic dysfunction, postural instability, falling, dementia, and symptoms related to the administration of other drugs. Theincreasingly diverse possibilities in the therapy of PD, and the many side effects and complications of therapy, require reliable standards for patient care that are based on current scientifi c knowledge.This chapter provides these scientifi cally supported treatment recommendations.If the level of available evidence is only Level IV, i.e if the evidence is based on expert opinion and scientific evidence is lacking and therefore the rating of recommendation is below C, best practice is recommended (GPP).
