JACC：心肌梗死术后住院多久更好？

住院时间长短是医疗护理效率的一个重要的量度，这必须小心谨慎地与医疗治疗相平衡。缩短住院时间主要是降低经济方面的压力。但是，更长的住院时间考虑到了提高以病人为中心的治疗并过渡到门诊部，这可以降低再住院率并提高医疗结果。对于经皮冠状动脉介入治疗的ST段抬高的心肌梗死病人来说，住院时间的长短对疾病的治疗有着很重要的影响。这一类病人需要住院监测心律失常和心肌梗死的并发症。

科研人员调查了国家心血管数据登记处的资料，以分析住院时间长短对接受经皮冠状动脉介入治疗的心肌梗死病人术后结果的影响。共调查了33920例病人。其中9135例（26.9%）住院少于3天，15704人（46.3%）住院4-5天，住院超过5天的有9081人（26.8%）。主要的监测指标是病人全死因死亡率，MACE（死亡，心肌梗死再住院或者意外血管再生）。住院时间长的病人普遍是年龄较大的女性且有更大的并发症，包括心原性休克和多血管疾病。住院较短的病人通常有着更高的射血分数和单血管疾病。短期住院和中期住院相比，三十天内全死因死亡率或主要不良心脏疾病并没有显著差异。住院1-2天的病人相较于3-4天的病人30天内死亡率和主要不良心脏疾病有着显著升高。

此次研究结果表明，手术48h之内出院的术后结果和住院4-5天的结果相似。早但不是非常早（<48h）出院可能更为安全。
