J Endod:常规根尖X线影像对诊断根分叉穿孔治疗后修复的准确性

根尖X线影像(PR)是在临床实践中用来进行专业诊断的工具。此种方法存在一定的限制,并且对其评估根分叉穿孔和治疗后软硬组织修复过程的价值存在质疑。因此,这篇研究的目的是通过黄金标准-组织病理学分析检测实验性穿孔诱导后根分叉区域的吸收以及穿孔治疗后的修复情况,评估PR作为检测诊断方法的准确性。

研究对30只比格犬的根分叉穿孔分别施以Biodentine (Septodont, Saint-Maur-des-Fossés, France), ProRoot White mineral trioxide aggregate (Dentsply Tulsa Dental Specialties, Tulsa, OK)或gutta-percha充填,使用PR和组织学检验。计算敏感性、特异性、预测值和准确性。

结果显示,PR可以检测到根分叉穿孔修复和吸收的比例分别为55%和48%。组织学分析显示,根分叉穿孔修复和吸收的比例分别为66%和21%。PR检测修复的敏感性和特异性分别为0.84和1,而对于吸收的检测值分别为0.43和0.65。PR对于根分叉穿孔修复和吸收的诊断准确性分别为0.89和0.72。PR对于根分叉穿孔修复的诊断性(true positives + true negatives/total)明显高于吸收。

结论:PR不适合用于根分叉穿孔修复后再吸收过程的检测。当怀疑可能存在根分叉穿孔修复治疗失败的结果时,研究鼓励使用数字化影像。
