背景:脑血流自动调节在脑出血患者中至关重要。动态脑血流自动调节在急性脑出血中可能改变;然而,动态脑自动调节功能的时间过程及与临床因素和预后的的相关性知之甚少。方法:招募43例急性幕上脑出血患者(53.7±10岁,30例为男性),在发病后1到2天,4到6、10和12,发病后30天进行检测。分别应用多普勒和伺服控制体积描记器无创记录脑血流速度和动脉血压。应用传递函数分析得出的调节参数包括相位差(PD)

背景:脑血流自动调节在脑出血患者中至关重要。动态脑血流自动调节在急性脑出血中可能改变;然而,动态脑自动调节功能的时间过程及与临床因素和预后的的相关性知之甚少。

方法:招募43例急性幕上脑出血患者(53.7±10岁,30例为男性),在发病后1到2天,4到6、10和12,发病后30天进行检测。分别应用多普勒和伺服控制体积描记器无创记录脑血流速度和动脉血压。应用传递函数分析得出的调节参数包括相位差(PD),增益,和脑血流速度的恢复率。与健康对照组相比,临床因素和90天预后的相关因素。

结果:随着时间的推移,受影响半球和不受影响半球之间的PD没有差异。在1至2天、4至6天、10至12天,双侧大脑半球PD显著降低(表示动态脑血流自动调节受损),30天后,恢复。1至2天双侧PD降低与当时格拉斯哥昏迷量表评分较低相关。4至6天,受影响侧PD减低是90天不良mRS评分的独立预测因子。

结论:幕上脑出血患者,双侧动态脑自动调节功能障碍至少持续10到12天,并在一个月内恢复。PD值与急性脑出血期临床状态相关,4-6天受影响侧PD是临床预后的独立影响因子
