血液中有两种类型的胆固醇:低密度脂蛋白(LDL)携带的所谓“坏”胆固醇LDL-C和所谓的“良好”胆固醇HDL-C高密度脂蛋白(HDL)。

已经证明降低LDL-C(例如通过他汀类药物)可降低心脏疾病和中风的风险,但HDL-C的因果作用并不明显,即使观察性研究显示HDL-C与心血管风险之间呈负相关疾病。

胆固醇酯转运蛋白(CETP)在不同的脂蛋白之间转移胆固醇。阻断这种提高HDL-C水平以及引起血脂变化的蛋白质是预防和治疗心血管疾病的潜在重要方法。改变CETP活性的遗传变异体可以模拟靶向该蛋白质的药物的一些作用,并且可以用于评估这种胆固醇修饰治疗的潜在益处和危害。

牛津大学,北京大学和中国医学科学院的研究人员对15万在中国加拿大生物库(CKB)中国成年人的CETP遗传变异体(包括东亚特异性功能变体)的健康影响进行了研究。经过10年的随访,5000多名研究参与者患有冠心病,19,000人中风。

研究人员发现,CETP遗传变异可显着提高HDL-C水平,但不会降低LDL-C水平,也不会降低心脏病和中风等心血管疾病的风险。对动脉粥样硬化斑块和动脉厚度,或其他疾病如糖尿病和肾病也没有影响。然而,该研究发现,眼部疾病风险增加是CETP抑制的一种可能的不利影响,这一发现得到了东亚和西方人群其他遗传学研究的支持。

在这项改变CETP活性的遗传变异的研究中,升高HDL-C而不降低LDL-C不会对心脏病或中风造成益处。该研究结果补充了来自最近REVEAL临床试验的CETP抑制剂anacetrapib的研究结果,该研究发现CETP抑制剂对心脏病的有益作用更可能是由于降低“坏”胆固醇LDL-C而不是提高“良好”HDL- C.2。

新的治疗方法是通过探索导致疾病的生物途径而发现的,但可以通过药物进行修饰。从基础生物学到大规模人类随机试验的过程是漫长而昂贵的:单一产品上市的估算成本超过20亿美元。利用遗传数据预测新疗法的好处和危害可以降低成本,改善药物开发渠道。

在将来进行大规模临床试验之前,这种方法可能会变得更加广泛地用于检查疾病中涉及的生物途径的因果性质。已知有数千种功能性遗传变异体可能代表不同生物途径中的潜在药物靶标,并且研究小组正在使用相同的方法来评估许多其他重要的治疗靶标。

研究作者牛津大学的Iona Millwood博士说:“我们的研究有助于阐明不同类型胆固醇的作用,并提示通过阻断CETP活性而不降低LDL-C来提高HDL-C的水平,对心血管疾病没有任何主要的益处。”

来自北京大学的研究共同作者李明教授说:“CKB是一个强有力的合作研究资源,不仅可以告知生活方式因素(如吸烟,饮食和身体活动)与健康的关联性,还可以指导药物研发。”
