近日，呼吸疾病领域权威杂志Eur Respir J上发表了一篇研究文章，该研究的目的是调查以家庭为基础的测定用力肺活量（FVC）和呼吸困难作为特发性肺纤维化（IPF）临床终点的可行性、可靠性以及分析影响。IPF患者使用移动手持肺活量仪和自填式呼吸困难问卷每周进行一次的家庭为基础的FVC和呼吸困难评估。研究人员估计了受试者FVC和呼吸困难的每周变异，以及对使用传统的基于诊室的间隔测量或移动每周评估的假设的24周的临床试验的样本量大小进行了模拟。该研究总共有25名患者入选。平均坚持每周评估超过24 周的受试者大于90%。相比于只是使用基线和24周测量指标变化评估的患者，FVC每周评估导致精度和效能增加。例如，假设24周的临床试验，以FVC为主要终点需要951例每周使用家庭肺活量仪相比于3840例只在第1周和第24周使用诊室肺活量仪测量的患者。重复测量能够降低临床试验样本量受数据相关结构的影响。由此可见，家庭监测可以提高研究终点评估的精确度，使IPF治疗在临床试验中的效率更高。原始出处：Kerri A.
