大规模研究发现,补充维生素D,降低痴呆风险

维生素D的主要来源是紫外线,它在维持骨骼和肌肉骨骼健康方面有着公认的作用,还可以帮助抵御感染和癌症。近几年来,研究人员发现维生素D水平与痴呆症之间存在很强的相关性。

阿尔茨海默症(AD),又被称为痴呆症,是一个具有高经济和社会负担的全球健康问题。全球约有5740万痴呆症病例,每年新增约有1000万例,在中国,有上千万患者,位居全球第一。尽管科学界投入了许多精力,但目前仍然没有十分有效的治疗方案来预防或治愈。

维生素D的主要来源是紫外线,它在维持骨骼和肌肉骨骼健康方面有着公认的作用,还可以帮助抵御感染和癌症。近几年来,研究人员发现维生素D水平与痴呆症之间存在很强的相关性。

2023年3月1日,加拿大卡尔加里大学、英国埃克塞特大学的研究人员在" Alzheimer’s & Dementia:Diagnosis,Assessment & Disease Monitoring "期刊上发表了一篇题为" Vitamin D supplementation and incident dementia:Effects of sex,APOE,and baseline cognitive status "的研究论文。

该研究显示,补充维生素D可能有助于预防痴呆症,与更长的无痴呆生存期有关。并且,服用维生素D组,患痴呆症的风险降低了40%。

在该研究中,研究人员分析了美国国家阿尔茨海默病协调中心的12388名无痴呆症参与者,平均年龄为71岁,其中4637人服用了维生素D,7751人没有服用,研究人员分析了服用维生素D与痴呆症之间的关系。

总的来讲,2696名参与者在10年内发展为痴呆症,其中未服用维生素D组发现2017例痴呆,服用维生素D组发现679例痴呆。

对于未服用维生素D组发展为痴呆的患者,82.6%为阿尔茨海默症,4.8%为路易体痴呆,2.4%为行为变异额颞叶痴呆,2.1%为血管性痴呆;而在服用维生素D组这一比例为,80.9%,4%,2.4%,0.6%。

研究发现,与没有补充过维生素D的人相比,补充过维生素D的人痴呆风险降低40%。

维生素D暴露和无痴呆生存率

对于不同性别,研究发现,虽然维生素D对所有人群都有效,但与男性相比,女性的效果明显更大。与不补充维生素D的女性相比,补充维生素D的女性痴呆风险降低49%。而男性这一比例仅为26%。

此外,与报告有轻度认知障碍迹象的人相比,认知正常的人的影响更大。研究人员表示,这一发现强调了在病程早期进行干预的重要性,在认知能力下降开始之前尽早补充可能特别有益。

研究人员还分析了APOE-4基因的影响,发现维生素D对不携带APOE-4基因的人的影响也明显更大。

暴露维生素D与性别、认知状态、APOE-4等对痴呆风险的作用

早期的研究显示,低水平的维生素D与较高的痴呆风险有关,维生素D参与大脑中淀粉样蛋白的清除,维生素D可能有助于保护大脑免受tau蛋白的堆积。

研究人员表示,鉴于受痴呆症影响的人数越来越多,预防痴呆甚至延缓其发作至关重要。研究人员正在进一步完成随机对照试验,通过临床试验来证明维生素D是否真的如此。

总之,这项新研究增加了越来越多的证据,表明维生素D有可能成为预防痴呆症的有效药物,特别是对于认知能力下降的高风险人群。
