TTDE和MCE在冠脉微血管功能障碍中的应用与评估

(一)经胸多普勒超声心动图(Transthoracic Doppler echocardiography,TTDE)


通过测量心表大冠脉血流速度参数及其储备反映冠脉微血管功能。前降支距离胸壁较近,有经验的医生应用TTDE测定其血流速度的成功率可达90%以上,如若联合超声造影剂,成功率甚至有可能高达100%,是目前TTDE评估冠脉微血管功能首选窗口。右冠状动脉后降支血流信号可于心尖两腔心切面观左室下壁中部后方的后室间沟内探及,检出成功率约为50%-90%,可作为TTDE评估冠脉微循环功能的第二个窗口。因声窗问题,左回旋支血流速度测定较为困难,成功率仅约50%左右,故临床上较少选用左回旋支来评估冠脉微循环功能。TTDE测量冠脉血流储备的重复性研究表明,同一研究者重复测量及不同研究者之间的测量变异率不超过5%。 

1.TTDE评估冠脉无复流现象 


既往研究表明,急性心肌梗死再灌注治疗后“无复流”比例高达50%。微血管功能障碍导致微循环阻力增加,TTDE可观察到舒张期冠脉血流减速时间缩短(diastolic deceleration time, DDT)、舒张期峰值血流速度增加、收缩期峰值血流速度下降。其中DDT是较为常用指标。一般认为DDT>600 ms代表冠脉微血管闭塞。但实际上冠脉微血管功能障碍并非是一个“全或无现象”,其中无复流现象只是冠脉微血管严重闭塞的结果。当DDT<190 ms则有可能提示无存活心肌了。在临床上我们可能见到更多的是“慢血流现象”,不同的DDT变化也许代表不同程度的“慢血流现象”。 

2.TTDE评估冠脉血流储备 

高血压糖尿病、心脏X综合征、心肌病等,在心表大冠脉造影未见异常情况下,可出现冠脉血流储备的异常。刘俐等应用TTDE测量1级高血压患者的CFR为3.02±0.64,明显低于正常对照组(3.77±0.58,P <0.05),提示轻度高血压患者即可出现冠脉微循环功能障碍。Kawata 等在研究中发现,无冠心病合并症的2 型糖尿病患者中,CFR<2.5 的患者不良事件发生率明显高于CFR>2.5 的患者。目前TTDE-CFR的正常界值尚未统一,可参考PET-CFR研究结果:CFR<1.5提示明确的冠脉微血管功能(CMD)异常;CFR>2.6提示无明显CMD;如1.5

3.TTDE用于冠脉微血管功能障碍的病理生理机制研究
 

Sezer M等应用TTDE观测前降支平均峰值血流速度、冠脉血流速度储备、冠脉微血管阻力发现:与正常对照组比较,糖尿病病程短(<10年)患者,因静息微循环阻力降低(3.69±0.86 vs 4.34±0.76, P=0.003)导致基础状态前降支平均峰值血流速度增加(26.50±5.6 versus 22.08±4.31, P=0.008),最终冠脉血流速度储备低于对照组。而对于病程超过10年的糖尿病患者,则缘于负荷状态下循环阻力增加(2.13±0.42 versus 1.69±0.39, P<0.001),导致负荷状态下峰值血流速度下降,最终冠脉血流速度储备低于对照组。提示短病程(<10年)糖尿病患者基础状态即出现微血管功能调节异常,而长病程(>10年)患者多因出现心肌结构异常而导致微循环灌注障碍。说明两者在微循环障碍机制方面存在明显不同。 

(二)心肌声学造影(Myocardial Contrast Echocardiography, MCE)是利用一种大小及变形性与红细胞相当的微气泡作为血流示踪剂,该造影剂经静脉注射后通过肺循环抵达冠脉微循环,从而实现心肌血流成像。故MCE可用来在跳动的心脏上评估冠脉微循环功能。虽然该临床适应证尚未获得FDA批准,但因为临床左室声学造影过程中可同步观察到心肌灌注情况,故在有条件的医院,仍有望开展MCE研究从而为临床医生提供更多的额外诊断信息。 

1.MCE动态观察“危险区”范围 

冠脉急性闭塞后,因为侧支循环的存在,梗死区周边存在一个缺血区(“危险区”)。该区域血流灌注特点为从梗死中心向外血流速度梯度增加直至到达正常心肌灌注区。但该范围的大小如何评估,一直是个临床难点。我课题组应用高机械指数间断触发成像模式成功观察到“危险区”的动态变化过程,从而为冠心病的治疗及预后评估提供了重要参考。 

心尖球形综合征在临床上酷似急性心肌梗死。但此类病人,MCE较少见到大片心肌灌注缺损区,而多表现为正常或轻度异常灌注。与典型心肌梗死有明显区别。MCE可用于两者快速鉴别诊断的参考。 

2.MCE评价再灌注治疗疗效 

通过目测定性观察梗死区再灌注超声造影强度,或以正常区为参考标化后的梗死区造影强度可有效评估再灌注疗效。Joye等对16例急性心肌梗死溶栓再通病人分别行MCE及铊201静息成像。结果显示MCE判断存活心肌的敏感性、特异性、准确性不劣于核素。Andrassy研究表明,以梗死区标化视频强度>0.53为标准,MCE判定再灌注治疗术后存活心肌的敏感性、特异性、准确性分别为85%、66%及76%;以标化视频强度>0.40为标准,MCE判定再灌注治疗术后存活心肌的敏感性、特异性、准确性分别为93%、65%及85%。我课题组以PET为对照,发现介入治疗后,梗死中心区如出现部分超声造影剂充填,即提示存活心肌的存在。如果进一步开展负荷超声造影(stress MCE),结合室壁运动参数的变化,将可以进一步提高超声判定存活心肌的能力。 

3.MCE定量血流 

超声造影时间-强度曲线(再灌注充填曲线)参数可直接用于心肌血流定量。大量研究结果表明显,超声造影定量血流与放射核素结果一致。但MCE不仅可以做到无放射性污染,还可以做到床边实时、多次重复检查。故对临床实践的潜在应用价值高于核素检查。 

4.MCE评估血流储备 

MCE评估心肌血流储备可通过定量或定性(半定量)方式实现。临床常用的负荷方式有药物(腺苷、潘生丁、多巴酚丁胺等)和运动(平板或踏车运动)两种。采取运动前后的超声造影图像,通过定量分析软件,获取负荷前后再灌注充填曲线参数(平台强度A及曲线上升斜率β,两者乘积反映心肌血流量),两者比值即代表CFR。该方法较为烦琐,临床上难于控制造影图像的漂移,故定量法更多地是用于动物实验研究。为简化临床操作,目前多用目测定性(半定量)方法,计数负荷后心肌灌注再充填过程所需要的心动周期数,如果在3-5秒内完全充填,即可初步认为该区域心肌微循环灌注正常。
