在观察性研究中,低血清25-羟维生素D (25[OH]D)浓度与较高的蛋白尿水平和较低的eGFR水平有关。在关于维生素D和2型糖尿病(D2d)的研究中,Kim Sun H等研究人员调查了补充维生素D对糖尿病前期人群肾脏结局的影响。

Effect of Vitamin D Supplementation on Kidney Function in Adults with Prediabetes: A Secondary Analysis of a Randomized Trial

2型糖尿病高风险的超重/肥胖的成年人被随机分至维生素D组(Vit D3, 4000 IU/天)或安慰剂组。中位治疗持续时间为2.9年(四分位范围:2.0~3.5年)。肾脏预后包括(1)肾病恶化:基线后连续两次随访的整体结果(KDIGO)风险评分(低、中、高、极高)提高,(2)eGFR和尿肌酐比值(UACR)的平均变化。

共招募了2166位受试者(平均60岁,体重指数 32 kg/m2,血清25(OH)D 28 ng/ml,eGFR 87 ml/min·173m2,UACR 11 mg/g,79%的个体有高血压),其中10%的个体为中、高或极高KDIGO风险评估。

各项研究终点结果

中位随访了2.9年,维生素D组和安慰剂组分别发生了28例和30例KDIGO恶化(风险比 0.89;95%CI 0.52-1.52)。维生素D组eGFR与基线相比发生的平均变化为-1.0 ml/min·1.73m2,而安慰剂组的平均变化为-0.1ml/min·1.73m2;两组间的差值为-1.0 ml/min·1.73m2维生素D组UACR的平均差异为2.7 mg/g (95%CI 1.2~4.3),安慰剂组为2.0 mg/g (95%CI 0.5~3.6);两组间的差值为0.7 mg/g (95%CI 1.5~2.9)。

综上,在未根据血清25(OH)D浓度进行预选的糖尿病前期患者中,补充维生素D并不影响KDIGO风险评分的进展,此外,补充维生素D对UACR或eGFR的变化也没有显著影响
