Hepatology:肝细胞癌的放射栓塞:在应答患者中可提高生存率

对单细胞癌的HCC患者,放射栓塞治疗可以提高生存率。在预测存活率方面,EASL坏死标准优于RECIST 1.1大小标准。放射栓塞治疗的目标应该是放射治疗,而不是单纯的阻止进展。

放射应答能预测肝细胞癌(HCC)的存活率吗?为此,研究者探索了治疗后放射应答预测总体生存率的能力。

超过14年的时间,948例肝细胞癌(HCC)患者采用放射栓塞治疗。排除标准:肿瘤转移、血管浸润、多病灶性疾病、Child-Pugh>B7、肝移植、肝切除等。研究中纳入134名符合标准的Child-Pugh≤B7、单细胞癌的HCC患者。

研究者使用界标(Landmark)分析和死亡风险方法,发现应答(使用欧洲肝脏研究协会定义的应答概念和实体肿瘤1.1[RECIST 1.1]标准中的应答评估标准)与生存有关。应答患者的生存时间与疾病稳定(SD)的患者和疾病进展(PD)的患者进行了比较。在每一个界标时间点,使用单/多变量生存分析进行相关分析。在3个月时,使用EASL评判标准,获得应答的患者相比未获得应答的患者生存的更久(P=0.002),然而,RECIST 1.1评判标准,认为两者没有显著性差(P=0.32)。在6个月时,使用EASL和RECIST 1.1评判标准,获得应答的患者相比未应答的患者生存的更久(P<0.001; P=0.021)。在12个月时,使用EASL和RECIST 1.1评判标准,获得应答的患者相比未应答的患者生存的更久(P<0.001; P=0.049)。在6个月时,使用EASL和RECIST 1.1评判标准,相比未获得应答的患者,获得应答的患者的死亡风险显著降低(P<0.001; P=0.0445)。应答患者相比SD、PD患者生存时间更久。在单/多变量生存分析中,EASL应答是3个月、6个月、12个月时重要的生存预测指标。
