根据Lawrence等的一项患病率研究(Arthritis Rheum, 2008),近2700万≥25岁的美国人被诊断为OA。既往有研究报道,膝关节OA是中老年人功能受限的主要原因,使得患者步行和爬楼梯困难。此外美国全国健康与营养状况评估调查(NHANES III)的数据显示,80%的OA患者行动受限,11%的膝关节OA患者需穿戴个人辅助器械。

尽管步行是中老年人常见的日常体育活动,但医学证据表明,2/3患有关节炎的美国成人每周步行时间不足90分钟。该研究试图明确增加步行运动量能否带来更好的躯体功能,如果答案是肯定的,那么每天需步行多少才能使膝关节OA患者进展为行动不便的风险降至最低?

为此,研究者们评估了多中心骨关节炎研究中1788例患有膝关节OA或具相关风险者每日的步行步数。研究者通过监控器测量了7天的步行情况,2年后评估了受试者躯体功能受限的情况。后者的定义为步行速度减缓,WOMAC(Western Ontario and McMaster Universities Arthritis Index)躯体功能评分高于28/68。

步行步数每增加1000步可使2年后躯体功能受限的发生率降低16%-18%。每天的步行步数少于6000步是区分进展为躯体功能受限者的最佳分界线。文章作者表示,步行是一项廉价的运动,尽管一般流行的观点认为步行目标为每天10000步,但本研究揭示,每天仅需步行6000步即可获得受益。研究者们鼓励患有膝关节OA或具相应风险的患者每天至少步行3000步,最终增加至每天6000步,以使行动困难的风险降至最低。
