J Hepatology:新冠肺炎患者肝脏异常与住院死亡率的关系

      由严重急性呼吸系统综合症冠状病毒2(SARS-CoV-2)感染引起的新冠肺炎目前还在世界范围内迅速传播,作为一种高度传染性疾病,新冠肺炎(COVID-19)首次被报道具有病毒性肺炎的特征,例如发烧,干咳和淋巴减少症,胸部CT成像有玻璃状混浊。此外,在COVID-19的患者中发现了肝脏的改变。COVID-19患者肝脏组织的病理学检查证实,COVID-19的肝脏受累特征为微脂肪变性,局灶性坏死伴淋巴细胞浸润和门静脉微血栓形成。肝脏异常的演变和临床意义以及乙型肝炎感染对COVID-19患者结局的影响尚不十分清楚。这项研究旨在探讨这些问题。

 

      本项大型回顾性队列研究纳入了2073例COVID-19感染患者,所有患者均在中国武汉获得了确诊和治疗,都进行了纵向肝功能测试,并通过多元回归分析确定了相关因素和死亡风险。研究人员再根据初步数据制定了预后列线图以预测COVID-19患者的生存情况。在倾向评分为1:3的情况下,比较有无乙型肝炎和无乙型肝炎的COVID-19患者的肝异常特征和预后。

 

      在2073名患者中,有1282名(61.8%)患者在住院期间出现肝化学指标的异常,而297名(14.3%)有肝损伤。死者在症状发作后很短时间内天门冬氨酸转氨酶(AST)和直接胆红素(D-Bil)的水平就出现了明显增加。AST([HR] 1.39; 95%CI 1.04–1.86,p = 0.027)和D-Bil(HR 1.66; 95%CI 1.22–2.26; p =0.001)水平是导致COVID-19死亡的独立危险因素。下图中的预测列线图显示出良好的区分能力,并且在预测和观察之间具有良好的一致性。患者的HBV感染并未增加与COVID-19相关的不良结局的风险。

      本项研究证实,入院时AST和D-Bil水平异常是COVID-19相关死亡率的独立预测因子。因此,对于住院的COVID-19患者,必须监测肝化学指标,尤其是AST和D-Bil的水平。
