血浆奇链饱和脂肪酸(OCFA)与2型糖尿病(T2D)风险呈负相关

血浆奇链饱和脂肪酸(OCFA)与2型糖尿病(T2D)风险呈负相关,还可作为乳制品脂肪摄入量的生物标记物。但是,它们在不同脂类中的分布和对糖尿病风险的影响仍不清楚。

本研究旨在探究含有OCFA的脂类与T2D风险的前瞻性关联,以及其饮食决定因素。

在欧洲癌症和营养前瞻性调查-波茨坦研究(n=27548)中,研究人员采用了一种嵌套的病例队列设计(亚队列:n=1248;T2D病例:n=820;中位随访 6.5年)。含有OCFA的脂质有三酰甘油、游离脂肪酸(FFA)、胆固醇酯(CE)、磷脂酰胆碱、磷脂酰乙醇胺、溶血磷脂酰胆碱、溶血磷脂酰乙醇胺、单酰基甘油和二酰基甘油。在性别分层Cox比例风险模型中,估计了含有OCFA的脂类与T2D之间的脂类特异性关联;并通过食物频率问卷调查了脂质和饮食摄入量之间的相关性。

研究人员观察到,不同脂类中OCFA的异质性整合: 三酰甘油、FFA、CE和磷脂酰胆碱对OCFA血浆丰度的贡献最大。单酰甘油中OCFA的相对浓度特别高,C15:0和C17:0对总OCFA丰度的贡献因脂类而异。

在女性中,几种含有OCFA的磷脂与T2D风险呈负相关(磷脂酰胆碱[C15:0] HR Q5 vs Q1:0.56, 95% CI 0.32–0.97;磷脂酰胆碱[C17:0] 每SD的HR 0.59, 95% CI 0.48–0.71;溶血磷脂酰胆碱[C17:0] HR Q5 vs Q1: 0.42, 95% CI 0.23–0.76)。在男性中,没有检测到磷脂具有统计学意义的负相关性,但溶血磷脂酰胆碱(C15:0)与较高的T2D风险相关(HR Q5 vs. Q1: 1.96, 95% CI 1.06–3.63)。

含C15:0和C17:0的脂类与基线时总脂肪摄入量的相关性

此外,CE(C17:0)、单酰甘油(C15:0)和二酰甘油(C15:0)与T2D风险呈负相关;FFA(C17:0)与女性T2D风险呈正相关。食用富含脂肪的乳制品和富含纤维的食物与含OCFA的脂质血浆水平呈正相关,而红肉则呈负相关。

综上所述,含有OCFA的脂类以脂类和性别特异性的方式与T2D风险相关;此外,它们在体内的水平还与多种食物的摄入量相关。
