JCO:循环肿瘤DNA作为弥漫大B细胞淋巴瘤早期结局预测指标

弥漫大B淋巴瘤患者的结局异质性较大,现有的方式无法预测治疗失败。JCO近期发表了一篇文章,研究治疗前及治疗中循环肿瘤DNA(ctDNA)预测患者结局的预后作用。

弥漫大B淋巴瘤患者的结局异质性较大,现有的方式无法预测治疗失败。JCO近期发表了一篇文章,研究治疗前及治疗中循环肿瘤DNA(ctDNA)预测患者结局的预后作用。

作者研究了在6个中心接受治疗的217例患者。在发现队列中,肿瘤治疗期间通过深度测序确定早期ctDNA变化的阈值。在两个独立的验证队列评估这些阈值。最后在已知的风险因素背景中评估ctDNA的预后价值。研究结果表明,98%的患者在治疗前可以检测到ctDNA,治疗前水平具有预后意义。在发现队列,ctDNA水平改变很快,一个周期后有2-log下降(早期分子反应(EMR)),两个周期后有2.5-log下降(主要分子反应(MMR))。在第一个验证队列,首次接受治疗并达到EMR或MMR的患者24个月结局更好。在第一个验证队列中,EMR还可以预测接受挽救治疗的患者24个月更好的结局。在第二个验证队列中进一步验证了EMR和MMR的预后价值。多因素分析表明分子反应是患者结局的独立预后指标。

文章最后认为,治疗前ctDNA水平和分子反应是侵袭性淋巴瘤结局的独立预后指标。这一结果有助于指导未来的个性化风险评估。
