Clin Cancer Res：晚期肾细胞癌免疫+靶向治疗后的无治疗生存期

采用纳武单抗联合伊匹单抗治疗的晚期肾细胞癌患者的无额外治疗下的生存时间明显更长

与靶向治疗相比，免疫检查点抑制剂 (ICI) 会产生独特的抗肿瘤反应和毒性模式。停止免疫治疗的患者可能可以享受一段不需要持续抗癌治疗的疾病控制期，但免疫治疗的毒性可能会持续存在。

该研究概述了接受免疫治疗的晚期肾细胞癌癌患者有无发生毒性的情况下的无治疗生存期（TFS）。

研究数据来自3期CheckMate 214试验，在该试验中，共有1096位初治的晚期肾细胞癌（aRCC）患者接受了纳武单抗联合伊匹单抗（n=550）或舒尼替尼（n=546）治疗。无治疗生存期定义：从治疗结束到随后再次进行全身治疗或死亡的时间。根据有无毒性反应进行分层。

两治疗组中低风险患者的TFS

随机分组后的第42个月时，纳武单抗+伊匹单抗组和舒尼替尼组分别有52%和39%的中低风险患者仍活着；分别有18%和5%的患者未进行额外的治疗。在有利风险患者中，纳武单抗+伊匹单抗组和舒尼替尼组分别有70%和73%的患者仍活着；分别有20%和9%的患者未进行额外的治疗。

两治疗组有利风险患者的TFS

纳武单抗+伊匹单抗组中低风险患者的平均EFS是舒尼替尼组的两倍（6.9 vs 3.1个月）；纳武单抗+伊匹单抗组有利风险患者的平均EFS是舒尼替尼组的三倍（11.0 vs 3.7个月）。在两个治疗组中，伴有3级及以上治疗相关不良反应的平均TFS只占一小部分时间：纳武单抗+伊匹单抗组 vs 舒尼替尼组：0.6 vs 0.3个月（中低风险）、0.9 vs 0.3个月（有利风险）。

综上，无论患者的风险高低，与舒尼替尼组相比，采用纳武单抗联合伊匹单抗治疗的晚期肾细胞癌患者无免疫副反应的无额外治疗下的生存时间明显更长。
