Neurology:胆碱酯酶抑制剂对认知衰退和死亡率的长期影响

近日,神经病学领域权威杂志Neurology上发表了一篇研究文章,该研究旨在明确胆碱酯酶抑制剂(ChEIs)是否与阿尔茨海默氏痴呆的认知功能衰退减慢以及严重痴呆或死亡的风险降低相关。

研究人员纳入了来自瑞典痴呆症登记处(SveDem)且在痴呆诊断后三个月内开始接受ChEIs治疗的阿尔茨海默氏痴呆患者,并与未治疗的阿尔茨海默氏痴呆症患者进行了比较。

在倾向得分匹配的队列中,研究人员使用混合模型评估了CHEI使用与通过简易精神状态评价量表(MMSE)分数评估的认知轨迹之间的关联,并使用Cox比例风险模型对严重痴呆(MMSE<10)或死亡作为结局进行了分析。

该研究纳入了11652名ChEI使用者和5826名非使用者。在平均5年的随访中,有255名受试者发展为严重痴呆,死亡病例达6055例(35%)。ChEI的使用与每次访视的较高MMSE相关(0.13 MMSE分/年;95%置信区间(CI)为0.06,0.20)。与不使用ChEI的人群相比,使用ChEI的个体死亡风险降低了27%(0.73; 95%CI为0.69,0.77)。加兰他敏与较低的死亡风险(0.71; 95%CI为0.65,0.76),较低的严重痴呆风险(0.69;95%CI为0.47-1.00)相关,并且是所有ChEIs中对认知下降影响最大的药物(0.18 MMSE分/年,95%CI为0.07,0.28)。

由此可见,ChEI与认知益处相关,并且随着时间的推移而持续存在,具有降低死亡风险的益处,这部分可以由其认知效应来解释。加兰他敏是唯一可显著降低罹患严重痴呆风险的CHEI。
