WJP:宣武医院揭示远隔缺血适应可降低烟雾病儿童缺血性脑血管事件的发生

远隔缺血适应可能是儿童烟雾病的一种有前景治疗方法。对于无症状的儿童烟雾病,长期RIC耐受性良好,可以降低缺血性脑血管事件的发生率,延长手术等待时间。

烟雾病(MMD)是儿童中风和短暂性脑缺血发作(TIA)的重要原因。远隔缺血适应(RIC)是一种强有力的脑保护干预手段。RIC的神经保护机制包括诱导缺血动物脑内血管生成和侧支形成,增强侧支循环,保护神经血管单元,改善脑血流量。World Journal of Pediatrics杂志发表了一项随机对照研究,探讨RIC治疗儿童烟雾病的安全性和有效性。

这项随机对照研究纳入46名4~14的无重建手术史的烟雾病患者,随机分配到RIC组或假RIC组,所有参与者均需完成RIC或假RIC治疗,每天两次,持续1年。所有入组患者均接受危险因素管理和生活方式指导,包括抗血小板治疗、控制血压、避免热餐、剧烈运动、吹管乐器和吹气球。RIC方案包括5个周期的双侧上臂治疗,使用自动设备进行5分钟充气至收缩压高于50mmHg,然后进行5分钟放气;假RIC方案包括5个周期的双侧上臂治疗,5分钟充气至30mmHg,然后进行5分钟放气。

主要结局是主要不良脑血管事件(MACEs)的累积发生率。次要结局包括缺血性卒中、复发性TIA、出血性卒中、血运重建率和随访期间使用患者整体印象(PGIC)量表评估的临床改善。

46例患者完成了最终随访(RIC组和假RIC组各23例)。3年随访期间,8例(17.39%)患者发生MACEs,其中假RIC组7例(30.43%),RIC组1例(4.35%)。未观察到RIC相关的严重不良事件。Kaplan-Meier分析显示RIC治疗后MACEs发生率显著降低[log-rank检验,P=0.021]。在3年的随访中,2例(4.35%)患者发生了缺血性卒中,4例(8.70%)患者发生了TIAs,2例(4.35%)患者因非脑血管事件进行了血运重建术。RIC组中只有1例患者出现TIAs,而其余患者均为假RIC组。基于PGIC量表,RIC组的临床改善率高于假RIC组(65.2% vs 26.1%,P<0.01)。治疗后脑血流动力学无统计学差异。

结果表明,RIC可能是儿童烟雾病的一种有前景治疗方法。对于无症状的儿童烟雾病,长期RIC耐受性良好,可以降低缺血性脑血管事件的发生率,延长手术等待时间。
