Neurology:中风后活动能力与主观幸福感

研究对中风患者预后生活幸福程度的影响因素进行了考察,但活动能力与预后幸福程度无关,中风患者预后的评估方法需要更为科学的方法

日常活动情况是众多卒中预后结果评分标准的重要组成部分。近日研究人员在全国性调查中考察了中风后患者的活动限度对其预后主观幸福感的影响。研究人员通过7项评估措施评估参与者的主观幸福感(主要终点,得分越高患者越幸福),包含情绪评估(快乐,无聊,充实的生活或不安)和自我价值实现(人生目的、自我接受和对环境的掌控)。通过11项日常生活/工具性生活项目考察评估患者活动是否受限。采用多变量线性回归评估健康、认知、心理和环境因素对幸福感的影响。研究招募了738名中风后患者,参与者年龄均大于65岁,57%为女性。研究发现,人口特征调整后,活动限制可适度预测患者的预后生活辛福感(预测度-0.49, 95% Cl -0.61--0.37)。但在完全调整模型中(R2 = 0.28),活动限制或者体力不能反映患者的生活幸福度。在最终的模型中预后程度的预测因子包括抑郁症状,咀嚼/吞咽困难以及疼痛。收入和执行功能与预后改善适度相关,但并发症和智能医疗方法不能改善患者预后幸福感。研究对中风患者预后生活幸福程度的影响因素进行了考察,但活动能力与预后幸福程度无关,中风患者预后的评估方法需要更为科学的方法。原始出处:Dar
