JAMA：富含血小板血浆注射不能改善慢性中部肌腱病患者跟腱功能

跟腱功能血小板血浆注射慢性中部肌腱病

对于慢性中部肌腱病患者，单次富含血小板血浆注射治疗对患者6个月跟腱功能评估无显著改善作用

慢性跟腱中部病变的特征是肌腱中部肿胀和疼痛，导致患者活动限制。2016年荷兰研究数据数据显示，每1000人中有5.2人患有慢性跟腱中部病变。目前治疗慢性中部肌腱病的方法包括运动、矫正器、电疗法和注射疗法，其中富含血小板的血浆(PRP)注射是重要的方法之一，但其临床疗效的证据十分有限。近日研究人员考察了PRP注射对慢性中部肌腱病患者VISA-A评分的影响。

来自24个医疗中心的240名经超声和或磁共振成像证实的慢性中部肌腱病患者参与研究，患者年龄在18岁以上，跟腱疼痛持续3个月以上，在2016年4月至2020年2月期间进行富含血小板的血浆注射或对照，其中富含血小板血浆注射组121人，对照组119人。研究的主要终点为6个月后 VISA-A评分，包含8个问题，涵盖疼痛、功能和活动3个领域。

患者平均年龄52岁，138名女性，221名（92%）患者完成了试验。在6个月的随访中，富含血小板血浆组的平均VISA-A评分为54.4，对照组为53.4（调整后的平均差异2.7）。与对照组相比，富血小板血浆注射组患者最常见的不良事件为注射部位不适（97例 vs 73例）、肿胀（56例 vs 52例）和瘀伤（48例 vs 49例）。

研究认为，对于慢性中部肌腱病患者，单次富含血小板血浆注射治疗对患者6个月跟腱功能评估无显著改善作用。
