近期,有研究人员探讨了人工耳蜗(CI)是否能抵消听力损失对认知能力的潜在影响问题。他们调查了重度和深度听力损失的老年人的认知能力是否在CI植入后12个月内提高到与听力正常的对照组相当的水平,并与年龄、性别和教育水平相匹配。

该项队列研究是在两个三级转诊中心进行的。研究包括29名年龄在60至80岁之间,患有成人时发病的重度至深度双侧感音神经性听力损失并有CI使用的患者(研究组),以及29名根据ISO-702 9:2000-01的标准曲线,经过年龄调整的听力能力的志愿者(对照组)。在进行CI前和CI后12个月,参与者完成了神经认知测试,包括全局认知、语言和形象记忆以及执行功能(注意控制、抑制和认知灵活性)的测试。

结果发现,与CI之前的情况相比,CI之后的12个月中研究组在整体认知方面的表现有了明显的改善,而言语情景记忆,图形情景记忆和执行功能方面的差异并不显著。此外,与对照组相比,研究组仅在全局认知方面的改善明显较大。对研究组在接受CI后的认知表现进行的非劣势测试显示,只有在全局认知、图形情景记忆(即时回忆)和注意力控制方面达到了与正常听力对照组相当的水平。全局认知的改善与CI后3个月的言语识别有显著关系,但与CI后12个月的言语识别无关。

综上所述,在进行CI一年后,与听力正常的同龄人相比,患有成人起始的听力损失的老年个体的认知缺陷只能得到部分的改善
