JAMA:血脂只知道“好”、“坏”胆固醇?这个指标与心血管疾病的关系非同小可!

Lp(a)在动脉粥样硬化中的作用非同小可。

越来越多的研究表明,相对于其他脂蛋白而言,脂蛋白(a)[Lp(a)]在动脉粥样硬化中的作用非同小可。近期,来自哈佛医学院麻省总医院(MGH)的三位专家在《美国医学会杂志》JAMA发表观点,讨论了Lpa与血管疾病(CVD)的关系、诊疗建议以及相关的新兴疗法。

Lp(a)概述

Lpa是一种类似于低密度脂蛋白(LDL)的脂蛋白,它的载脂蛋白(a)[apo(a)]通过一个二硫键与载脂蛋白B共价连接。目前的证据表明,Lp(a)是包括心肌梗死和缺血性中风在内的动脉粥样硬化性心血管疾病(ASCVD)的危险因素,因此在一级和二级预防人群中十分重要。同时Lpa也是主动脉瓣钙化引起狭窄的危险因素。

Lp(a)的结构

大约五分之一的人Lp(a)水平升高(>50 mg/dL)。Lp(a)水平主要与遗传有关,遗传因素的影响程度占75%-95%。与Lp(a)相关的风险增加是由于Apo(a)的促凝血作用增强,其致密性和促炎性作用归因于其氧化的ApoB相关磷脂。

Lp(a)降低CVD的风险

尽管测试降低Lp(a)水平将降低CVD风险这一假设的安慰剂对照试验尚未完成,但大量的临床前数据支持这一科学假设。对已完成的PCSK-9单克隆抗体CVD结果试验的分析表明,该类药物降低Lp(a)水平的效果比单纯降低LDL-C的效果更好,因此与CVD风险降低有关。

Lp(a)的测量

Lp(a)水平通常以mg/dL的整个脂蛋白(a)颗粒的质量或以nmol/L的apo(a)颗粒数来表示。偶尔,Lp(a)可表示为Lp(a)胆固醇(mg/dL)然而,由于现在的临床试验经常包括Lp(a)浓度较高的个体,因此必须提高标准化和一致性。

由于Lp(a)具有高度的遗传性,如果不使用已知会在初次测量后大幅改变Lp(a)水平的药物,通常没有必要重复检测。LPA位点的遗传变异可能提供与Lp(a)浓度相似的预后信息,但在已经有Lp(a)浓度的情况下,其增量的临床效用并不明确。

降胆固醇药物不能降低LDL,有时可能反映了由于Lp(a)浓度升高而导致的LDL-C估计的不精确性;为了解决这个问题,应从计算的LDL-C中减去Lp(a)-C。如果没有直接测定Lp(a)胆固醇,可以用Lp(a)质量乘以0.3来估计Lp(a)-C。

Lp(a)的治疗

目前还没有药物疗法被批准用于降低Lp(a)浓度和ASCVD风险虽然PCSK-9单克隆抗体可以降低Lp(a),但它们没有被批准用于达到LDL-C目标的人。烟酸可以适度降低Lp(a),但除了他汀类药物外,可能不会进一步降低ASCVD风险。人类遗传学研究预计,可能需要大幅度降低Lp(a)以实现临床结果的有意义改善。

相关指南推荐

目前的美国指南建议,在年龄为40至75岁、估计10年ASCVD风险为5.0%至19.9%的一级预防患者中,可将Lp(a)作为 "风险增加因素",以支持他汀类药物的使用。此外,欧洲指南建议对所有成年人进行广泛的一次性Lp(a)测量,以识别Lp(a)>180mg/dL(或>430mmol/L)的非常高危的个体。

这些人可能与那些杂合子家族性高胆固醇血症患者有类似的终生ASCVD风险,但与LDL-C不同的是,临床上有效的降低Lp(a)的疗法尚未得到前瞻性证实。尽管Lp(a)被认为是促血栓形成的,但阿司匹林是否能减轻Lp(a)相关的ASCVD风险尚未得到很好的研究,因此目前指南中不建议仅对Lp(a)升高的患者使用阿司匹林。
