抗骨质疏松用好三类药

骨质疏松症是以骨量减少、骨微观结构退化为特征、骨脆性增加以致骨折风险提高的一种全身性骨代谢障碍疾病。抗骨质疏松药物治疗可以减少骨质疏松相关性骨折的发生,目前临床上常用的药物有以下三大类。

骨质疏松症是以骨量减少、骨微观结构退化为特征、骨脆性增加以致骨折风险提高的一种全身性骨代谢障碍疾病。抗骨质疏松药物治疗可以减少骨质疏松相关性骨折的发生,目前临床上常用的药物有以下三大类。

骨矿化类。包括大家最熟悉的钙剂、维生素D及其衍生物。补钙过程中联合使用维生素D及其衍生物(含维生素D)可以促进肠道对钙磷的吸收,增加肾小管对钙的吸收以及抑制骨吸收,起到协同作用。也就是说,钙剂和维生素D及其衍生物合用,可使补钙效果事半功倍。

抑制骨吸收药物。补充钙剂是“开源”,那抑制骨吸收药物就是“节流”。这类药物包括二磷酸盐,即阿仑膦酸钠、帕米磷酸钠、依替膦酸二钠等。阿仑膦酸钠是当前治疗骨质疏松一线用药,它与矿化的骨基质结合,能降低骨转换、增加骨密度,有效缓解疼痛、预防骨折。其不良反应以胃肠道反应为主,一般可以耐受。

降钙素。降钙素具有抑制骨吸收、促进骨重建的作用,能抑制中枢、缓解疼痛,对伴有骨痛的骨质疏松效果较好,且不良反应较少。临床常用通过人工合成的鲑鱼降钙素,但单独使用降钙素可引起低血钙和甲状腺功能亢进,导致骨吸收和骨丢失增加,建议降钙素与钙剂合用。

其他药物还包含雌激素、骨合成促进剂等。雌激素对维持女性骨量和骨代谢起着重要作用,补充雌激素联合抗骨质疏松药物可有效治疗绝经后骨质疏松,但雌激素可能增加子宫内膜癌、乳腺癌等风险,不作为防治骨质疏松的一线用药。
