背景：在炎性肠病（IBD）中，嗜中性粒细胞聚集在发炎粘膜上，在发病机理中发挥了重要作用。CD177在嗜中性粒细胞中表达，在发炎过程中增加。然而，CD177+嗜中性粒细胞在IBD发病机制中的作用仍然难以捉摸。

材料与方法：使用定量RT-PCR，流式细胞术和免疫组化学法分析IBD患者外周血和肠粘膜中CD177的表达。分离CD177+和CD177-嗜中性粒细胞，通过RNA测序确定基因差异。用葡聚糖硫酸钠诱导CD177-/-和野生型小鼠结肠炎，以确定CD177+嗜中性粒细胞在IBD中的作用。

结果：与健康对照组相比，IBD患者外周血和炎症粘膜中的CD177+嗜中性粒细胞明显升高。RNA测序显示，来自IBD患者的CD177+和CD177-嗜中性粒细胞之间的差异基因表达与对细菌防御、过氧化氢和活性氧（ROS）的反应有关。与CD177-亚型相比，CD177+嗜中性粒细胞产生较低水平的促炎细胞因子（即干扰素-γ，白介素（IL）-6，IL-17A），但IL-22和转化生长因子-β水平较高，并且表现出增强的杀菌活性（即ROS、抗菌肽、嗜中性粒细胞的胞外捕获）。与野生型小鼠相比，CD177 - / -小鼠对DSS诱导发展了更严重的结肠炎。此外，通过CD177-嗜中性粒细胞产生的IL-22减少，CD177缺乏导致肠道屏障和抗细菌免疫受损。

结论：CD177+嗜中性粒细胞代表功能活化的人群，并通过增加杀菌活性和IL-22的产生，在IBD中发挥保护作用。靶向CD177+嗜中性粒细胞可能有益于IBD的治疗。
