如何处理支架术后FFR仍较低的患者?

当患者FFR值低于预期时,关键在于使用撤回压力轨迹来确定存在可纠正的问题。对于FFR撤回轨迹的追踪,重要的是在术中应保持稳定的充血过程,但真正实现平稳完整的维持效果很难。在Zandvoort等的研究中,40%的病例在输注腺苷期间出现充血效果的波动。最近推广的瞬时无波率撤回术的优势在于不要求充血效果,这一优势在与血管造影图像融合技术相结合时更为突出。如果出现某个病灶(图A),使用血管内超声和光学相干断层扫描有助于揭示患者低FFR的原因并改进治疗策略。但如果低FFR由弥漫性病变所致(图B),增加额外的支架置入并非最佳治疗方案,无法给患者带来长期获益。



综合影像学和生理学评估

冠状动脉支架置入术的目的是改善患者预后,而非单纯减轻冠状动脉狭窄症状。支架置入后,患者的临床结局将取决于是否残存缺血情况、总体残留解剖学/生理学疾病负担及其特点、支架置入的充分性、是否存在支架相关并发症、是否存在微血管疾病以及个体心血管危险因素等。单纯影像学评估或生理学评估,都不能完整提供上述所有信息,因此,临床医生需要摆脱“哪种技术更好”、“最佳临界值是多少”诸如此类的二分法思维模式,只有综合信息才能更好预测患者的风险并给予更适合的指导策略。此外,医生不能只着眼于生理评估所得的数字,而应窥一斑而见全豹,找出冠状动脉影像学画面背后的实际全貌。

JACC:血管内超声指导下冠脉支架植入预后更佳

本研究的目的旨在比较评估血管内超声(IVUS)指导下和血管造影指导下的药物洗脱冠脉支架植入(DES)的优点。本研究对1448名需要接受DES植入的患者随机1:1分成IVUS组和血管造影组,主要终点事件是术后12个月的靶血管衰竭(TVF),包括心源性死亡、靶血管相关心梗和靶血管再生(TVR)。经过12个月后,60名患者发生TVF(4.2%),其中IVUS组21名(2.9%),血管造影组39名(5.4
