氧化应激目前被认为是肾脏疾病的一个重要治疗靶点。然而,目前还没有生物标志物可用于临床诊断肾氧化应激。本研究试图研究硫氧还蛋白能否作为一种氧化应激依赖性急性肾损伤的早期生物标志物。尿硫氧还蛋白1是急性肾损伤(AKI)的氧化应激相关生物标志物,其快速检测系统可被开发为化学发光酶免疫检测系统,并得到了分析和临床验证。

分析评估显示,溶血性血红蛋白导致测量值异常高,高于可检测范围。然而,含有红细胞的尿液沉淀物并不影响测量结果。使用本研究提出的化学发光酶免疫分析法的检测在短短6分钟内完成,而传统的ELISA需要耗时 > 4小时。酸性尿<pH 6.0导致硫氧还蛋白1浓度明显低估。然而,酸性尿的影响是完全可逆的使用为pH调节开发的缓冲。AKI患者尿中硫氧还蛋白1升高,但不受包括低氧血症和心肌梗死在内的肾外氧化应激疾病或血清肌酐浓度相当的慢性肾脏疾病的影响。

这些结果表明,尿硫氧还蛋白1的化学发光酶免疫分析系统能够快速、特异地诊断与氧化应激相关的AKI。
