Eur radiol:神经根阻滞用哪种皮质醇?比较微粒与非微粒皮质醇的差异性

腰骶神经根病是周围神经病损中最常见的一种,大多数是由于腰椎间盘突出压迫神经根所致。本研究旨在对照CT引导下行微粒或非微粒皮质醇经椎间孔神经根阻滞的效果,并将结果发表在Eur radiol上。

腰骶神经根病是周围神经病损中最常见的一种,大多数是由于腰椎间盘突出压迫神经根所致。本研究旨在对照CT引导下行微粒或非微粒皮质醇经椎间孔神经根阻滞的效果,并将结果发表在Eur radiol上。

本回顾性研究比较了两组腰骶神经根病患者的预后,其中包括321例接受微粒皮质醇和173例接受非微粒CT引导下经椎间孔腰神经根注射的患者。微粒皮质醇使用时间为2009年10月至2014年5月,非微粒皮质醇自2014年5月开始使用。利用11分数值评定量表(NRS)在基线时、1天后、1周后和1月后进行收集疼痛水平。利用患者整体体验变化(PGIC)在相同时间点评估整体改善情况。利用卡方检验比较在两组患者中改善的比例。利用独立样本T检验比较NRS变化评分。

结果为,接受微粒皮质醇治疗的患者改善的比例在1周后(43.2 % vs. 27.7 %, p = 0.001)、1月后(44.3 % vs. 33.1 %, p = 0.019)要明显高于非微粒皮质醇。接受微粒皮质醇治疗的患者NRS变化评分在1周后(p = 0.02)、1月后(p = 0.007)要明显高于非微粒皮质醇。

本研究表明,微粒皮质醇在经椎间孔神经根阻滞治疗腰骶神经根病的效果要明显优于非微粒皮质醇。
