意大利一项研究表明，在肥厚型心肌病患者中，血浆N-端脑利钠肽前体（NT-proBNP）为死亡和心衰相关性事件的有效独立预测因素。论文7月22日在线发表于《美国心脏病学杂志》（Am J Cardiol）。

此项研究共纳入183例稳定性肥厚型心肌病门诊患者（年龄50±17岁，男性64%），并在测定NT-proBNP后进行了3.9 ± 2.8随访。主要终点为心血管死亡、心脏移植、心搏骤停复苏和适宜ICD干预。次要终点包括心衰相关性死亡或住院、进展至终末期疾病和卒中。

结果显示，NT-proBNP中位水平为615 pg/ml。NT-proBNP低、中、高三分位患者的主要终点年度发生率分别为0%、1.3%和2.1%（P=0.01）。多变量分析显示，仅NT-proBNP和左室充盈限制模式为主要终点的独立预测因素。NT-proBNP阈值810 pg/ml对主要终点而言具有最佳敏感性（88%），但特异性较低（61%）。NT-proBNP低、中、高三分位患者的次要终点年度发生率分别为4.6%、12.0%和11.2%。与NT-proBNP≥310 pg/ml相比，NT-proBNP<310 pg/ml与次要终点发生率减低75%相关，并且独立于年龄、左室流出道梗阻或房颤。
