在头部MR平扫的时候,FLAIR图像经常会在脑室系统内出现以下现象,有时候还很明显,是病变?还是伪影呢?您有没有迟疑过?



这种现象应该称为:脑脊液伪影

为什么会产生这种现象呢?

主要原因是脑脊液处于流动状态,且流动是双向的,由于脑运动的影响使得脑脊液流动形式复杂,呈搏动性运动。

FLAIR成像时,在反转脉冲和采集信号的时间间隔内,上方或下方的脑脊液流入成像层面,因此未能被有效抑制,从而产生信号。此外,脑脊液搏动、局部场强不均匀等亦可产生脑脊液伪影。

这种现象有什么特点呢?

部位:最常见于鞍上池、桥前池和脑室的孔处

年龄:儿童相对成人少见。

形态:侧脑室者一般位于孟氏孔区 , 呈椭圆形 , 三脑室者一般呈片状 , 导水管者多位于末端呈圆形 , 四脑室者常位于其上部呈小圆形或条片状 ;

其他:无占位效应 ,邻近无改变

怎么减轻这种现象呢?

建议把最小采集间隔从 2 改为 3 ,目的是增加反转脉冲厚度 ,在 2 倍采集间隔的扫描时反转厚度大约是成像层面的两倍厚 ,在 3 倍采集间隔的扫描时反转厚度大约是成像层面的三倍厚 。

加宽反转脉冲厚度可反转在反转时间间隔时流入成像层面的脑脊液 ,减少非反转的脑脊液流入成像层面时产生的伪影。
