Thorax:渐进式呼吸肌训练可增加四肢瘫痪者的吸气肌力

呼吸系统并发症仍是急、慢性四肢瘫痪者发病和死亡的主要原因。脊髓损伤引起的四肢瘫痪后呼吸肌无力会损害肺功能和咳嗽能力。特别是,在四肢瘫痪患者中,吸气肌力量已被确定为肺炎发生可能性的最佳预测指标。我们假设6周的渐进式呼吸肌训练(RMT)可以增加呼吸肌力量,同时改善肺功能、生活质量和呼吸系统健康。

双盲随机对照试验设计,纳入62名四肢瘫痪的成年人。每天进行两次主动或假RMT,持续6周。主要结果为最大吸气压力(PImax)测量的吸气肌力。次要结果包括肺功能、生活质量和呼吸系统健康。通过线性模型校正结果的基线值,获得组间比较。

 

结果显示,6周后,与假手术组相比,活动组的PImax改善更大(平均差异11.5 cmH2O(95% CI 5.6~17.4),p<0.001),呼吸道症状减轻(St George呼吸问卷平均差异10.3分(0.01~20.65),p=0.046)。生活质量(EuroQol-五维视觉模拟量表14.9分(1.9-27.9),p=0.023)和感知呼吸困难(Borg评分0.64(0.11-1.17),p=0.021)有显著改善。呼吸功能的其他测量指标没有明显改善(p=0.126-0.979)。

综上所述,该研究结果表明,渐进式RMT可增加四肢瘫痪者的吸气肌力,其幅度可能具有临床意义。测量基线PImax并为高危人群提供RMT可能会减少四肢瘫痪后的呼吸道并发症。
