Am J Cardiol:脂蛋白亚组分评估有助预测冠脉钙化

  此项研究共序贯性纳入410心血管中等风险患者。受试者平均年龄57岁,女性占29%,并且均接受冠脉钙化(钙化评分)CT评估。利用经验证的超速离心法对洗脱脂蛋白亚组分(低、极低、中、高密度脂蛋白亚型,脂蛋白以及优势LDL分布)进行直接定量。

  结果显示,共有297例(72.4%)患者伴有冠脉钙化。对传统危险因素进行校正后,B型LDL(低密度颗粒占优势)为冠脉钙化的独立预测因素[比值比(OR)4.46]。但对血脂异常进行附加分层之后,在血脂正常组中仅高密度脂蛋白亚组分2(OR 3.45)和“真正”LDL(OR 6.10)具有显著预测价值,在血脂异常组中仅脂蛋白(a)具有显著预测价值(OR 7.81)。
