将患者随机分配进行基于obinutuzumab的化疗或基于利妥昔单抗化疗的诱导治疗。接受维持治疗具有反应长达2年的患者在诱导治疗中接受相同的抗体治疗。主要终点是研究者评估的无进展生存期。

结果,共有1202名滤泡性淋巴瘤患者随机分组(每组601例)。中位随访34.5个月(范围0-54.5)之后,中期分析显示,与基于利妥昔单抗的化疗相比,基于obinutuzumab的化疗患者疾病进展、复发或死亡的风险明显较低(3年无进展生存率80.0%vs 73.3%;进展、复发或死亡危险比为0.66;95%置信区间[CI],0.51〜0.85;P = 0.001)。在独立评估的无进展生存期和其他时间到事件终点方面也有类似的结果。两组的反应率相似(obinutuzumab组为88.5%,利妥昔单抗组为86.9%)。不良事件发生方面,obinutuzumab组3〜5级不良事件的发生率较利妥昔单抗组更为频繁(74.6%vs. 67.8%),严重不良事件发生率为也是如此(46.1%VS. 39.9%)。两组导致死亡的不良事件发生率相似(obinutuzumab组为4.0%,利妥昔单抗组为3.4%)。最常见的不良事件是输注相关事件,obinutuzumab组353/595例(59.3%; 95%CI,55.3至63.2),利妥昔单抗组292/597例(48.9% ; 95%CI,44.9〜52.9; P <0.001)。恶心和中性粒细胞减少很常见。obitutuzumab组35例患者(5.8%),利妥昔单抗组46例患者(7.7%)死亡。

总之,该研究发现,基于obinutuzumab的免疫化疗和维持治疗较基于利妥昔单抗疗法患者的无进展生存期更长。但是基于obinutuzumab的化疗,较严重不良事件的发生更为常见。
