爱吃红肉小心心脏病！Nature子刊：红肉吃多了心血管疾病找上门

该研究增加了我们对肠道微生物群参与和红肉摄入及CVD风险相关的营养代谢的理解。

古有大口吃肉、大口喝酒的英雄好汉；今有饭桌上无肉不欢的肉食爱好者。但是现在可要敲黑板了，肉吃多了不好！一项近日发表在Nature Mcrobiology上，题为“The microbial gbu genecluster links cardiovascular disease risk associated with red meat consumptionto microbiota L-carnitine catabolism”的研究表明：爱吃红肉会提高患心血管疾病的风险！
 
红肉是营养学上的词，指的是在烹饪前呈现岀红色的肉。例如猪肉、牛肉、羊肉、鹿肉、兔肉等等哺乳动物的肉都是红肉。早在2017年，红肉已被世界卫生组织国际癌症研究机构列为2A类致癌物。

文章的主要通讯作者Stanley Hazen教授在之前的研究中发现，富含肉碱的红肉和其他动物产品在进入肠道被消化时，肉碱会被转化为γBB（γ-丁基甜菜碱），然后转化为TMA（三甲胺）。这期间还会产生一种名为氧化三甲胺（TMAO）的副产物，也正是TMAO会增加患心脏病和中风的风险。

 而本次最新研究发现为明确摄入富含肉碱的红肉，在特殊肠道微生物的作用下将肉碱转化为促动脉粥样硬化和血液凝块的过程，提供了更为确凿的证据。
 
首先研究人员利用近3000名患者的临床数据研究了空腹血浆γBB水平与心血管疾病（CVD）风险之间的关联，并且在此期间调整了对传统心血管患病危险因素的影响。（年龄、性别、LDL-胆固醇、HDL-胆固醇、血压、高血压治疗、糖尿病和吸烟）、高敏 CRP、肾功能（肌酐）和 BMI结果发现：血浆 γBB 水平的增加与 CVD、冠状动脉疾病(CAD) 和外周动脉疾病 (PAD) 的患病率呈剂量依赖性相关，即使在调整了传统风险因素、C 反应蛋白 (CRP)、体重指数(BMI) 后也是如此。

紧接着，为了表征人类肠道微生物 γBB → TMA 的转化，研究者们使用粪便样本（n  = 50）在有氧和厌氧条件下监测γBB → TMA 的转化：通过LC-MS/MS 定量，在有氧或无氧条件下接触d9-L-肉碱24小时后，50个不同的人类粪便群落的肉碱消耗量和d9-TMA 产量。并在进一步的研究中观察到d9 -γBB 与来自杂食动物的粪便微生物群落如嗜温厌氧菌E. timonensis一起完全消耗了d9 -γBB，而来自纯素食者的那些不能消耗任何氘代底物。也就是说，引入的肠道细菌 E. timonensis 完成了肉碱向TMAO的转化，提高了TMAO水平，增加了凝血风险。

为了鉴定E. timonensis 中负责γBB → TMA转化的基因，研究者们使用RNA测序 (RNA-seq)评估了E. timonensis的转录组。分析显示在γBB 存在下，总共有6个相邻基因被显著诱导，将该基因簇命名为γBB利用（gbu）基因簇。在鉴定出gbu基因簇后，又使用组合克隆和功能分析来研究γBB → TMA 转换所需的基因。最终四种基因被鉴定为非天然大肠杆菌宿主中TMA生产所必需和充分
的：gbuA、gbuB、gbuC和gbuE。

最后，由于富含红肉的饮食与心血管死亡率升高有关,研究人员于是检查了粪便gbuA丰度与膳食蛋白质来源（包括红肉）和血浆TMAO水平之间的关系。共113 名健康参与者食用了在代谢厨房准备的不同蛋白质来源（红肉、白肉、非肉）的低热量饮食。所有饮食中，gbuA的粪便丰度与所有检查对象的配对血浆 TMAO 水平显着相关；无论饮食顺序如何，与红肉饮食相比，非肉类饮食后受试者的粪便gbuA丰度总体较低。因此，可以通过饮食结构来降低心血管疾病的发生。

综上，目前的研究增加了我们对肠道微生物群参与和红肉摄入及CVD风险相关的营养代谢的理解。并且研究表明肉碱→TMA的转化是通过肠道微生物群落介导的多步生物化学转化：即使在保持纯素/素食饮食的同时，也可以诱导依赖肠道微生物群的γBB→TMA 转化，导致致动脉粥样硬化和促血栓形成代谢物TMAO的形成增加。最后，gbu基因簇或许可以成为一个新的治疗心血管疾病的靶点。
